Health Review and Questionnaire
You may know it's essential to nurture yourself but good intentions often fall by the wayside. Complete the following questions and begin your journey of health and wellness. If you prefer, you can

the Health Review to your desktop and print a hard copy. The First Step Take time to explore what is really important to you about a healthy lifestyle. The obvious answer is to have a good quality of life. But please dig a little deeper. Ask yourself: "What's important to me about a good quality of life?" Is it spending time with family, traveling, pursuing creative projects? Connect with the
value
that supports the desire for good health. Your value helps motivate you to make necessary changes and let go of bad habits. For me, it's all of the above and one thing more. It may seem a bit frivolous but it motivates me and keeps me on track. What is it? I like wearing a size 10. Vain? Maybe. The value? It has to do with being my best. Whatever, it works! Now, it's your turn. Complete the questionnaire, taking time to reflect on question 1. - (a) What is important to you about a healthy lifestyle? Distinguish something specific that has meaning to you, such as travel, being the best grandparent, exploring your creativity.
(b) To keep you on track, put a picture or symbol that represents your answer to (a) in a prominent place, such as your fridge, where you will see it daily.
